#19 xml output should provide an id for annotation tags

For both "classMention" and "slotMention" they have an
attribute "id" with an assigned value, but there is no
id attribute for "annotation". How about <annotation
id="project_Instance_###">.

This should be straightforward to implement and will
probably be in the next release.
