#42 Issues with installer

I've added a very basic, primitive installer to the VS project.

Some of the current issues:
- the filename installed is "familyshow.exe", needs to be "kithandkin.exe". Will require changing the assembly name in the project, and all references. Or rename the .exe?
- rename familyshow.chm to kithandkin.chm, or don't install it
- Needs to create a desktop shortcut pointing to the application
- Needs a different icon?
- The release version of the setup project creates "setup.msi", whereas the debug version creates "kithandkin_v1.0beta.msi".
- Could tweak the appearance of the Wizard dialogs. See the attached "Family.Show V4" installer for ideas. E.g. an icon, change the license, samples are optional, etc.
- On my 64-bit machine, the installer put KithAndKin in "Program Files(x86)", not in "Program Files". Did I pick a bad setting? We aren't targeting x86 ...
- Confirm the dependencies for the setup: currently is ".NET 4.0 client" ?
