#547 Show category in default template

The following feature was requested by our beta tester Roswitha Lauterbach, see her email from 17-Apr-2010:
The default templates for question and answer, especially those created in the wizard, should show the category to avoid ambiguities.
Example for such an ambiguity: in an English-German vocabulary trainer, the question "still" could be German with the answer "silent", or it could be English with the answer "noch".
