#4 Toolbar icons in English

This patch is not expected to be incorporated into the
standard Kafenio (or yes, depending on the choice of
Kafenio project management.) It contains a replacement
for the icon pack standardly delivered with Kafenio.
The changes to the standard pack are
- icons copy, cut, and paste have been replaced with
iconsfrom Crystal Icons set
- icons bold and italic have been replaced with "B"
and "I" icons from java icon set. The patch bears no
copyright; the single icons have their respective
copyright or licences; they are all free for use.
