It would be nice if importing new patterns did not change which
existing patterns I have disabled .

Could you describe a bit the actual sequence of actions you did? I just tested
    with 1.14:

    1. Disable one of the patterns (in this case the "(?i)virus" in the Header tab)
    2. Delete one of the patterns in my To/CC tab
    3. Save the patterns (cmd-S)
    4. Double-click on my backup oldPatterns.jpp
    5. Import the just-deleted To/CC pattern, but not the disabled "virus"
    pattern
    6. The "virus" pattern remains disabled.

I do get the same behavior as you . Previously I did not notice that the
majority of the ones imported were "Changing the comment/active
    state".

My functionality request changes then . By default do not have the
import "check mark" the changes that are comment/active state changes
    .

Aside , it would be excellent to separate comment and active state
changes into two distinguishable types of changes.
