Two quick bugfixes to get v2.3.2 ready:

2002-02-22:
 - Refixed bug #517717 so MTMcreateStyleSheet() doesn't barf when the
   Mozilla quirk style element isn't present.
 - Fixed a part of the output code for non-DOM-capable browsers which
   made the first parts of the document not being output correctly.
