I have extracted the code but there isn't any documentation on the installation.

When I tried installing it does not work.

Ah, the htaccess file needs to be rename to .htaccess

This is also using some demo database that does not look like it is installed or comes with this package.

Also if you could provide the directory that this app is looking for to upload the files that would be great.
