#550 AbstractInstancePool is eating Exceptions

If AbstractInstancePool is unable to instantiate the bean class, there are cases where it fails to propogate the real cause when it wraps the exception in EJBException.

Additionally, it does not log its findings before throwing EJBException, so the only way to discover this Divine Intervention.

This patch fixes both of these errors.
