Parser:
  * Add a nasty hack involving a new "SPECIAL" state in the post-processor
    to work around some paragraph parsing that is breaking due to a
    previous change to support __TOC__ tags in tables.
  * This approach will need to be revisited - paragraph parsing is
    currently far too complex.
