#438 author/editor column

A combined author/editor column in the table would be nice, as some books have only an editor.

The column should list the authors and the editors if the author field is empty.

Thanks for this suggestion! I am adding the option to enter columns as "field1/field2/field3" where the fields are used successively as fallbacks if the previous fields are empty.
