#124 Inconsistent behavior of Add button in Project Window

If you have a document highlighted in the project manager and click the add button, the existing document is replaced in the project manager. It is unclear if the links are properly handled. This is behavior inconsistent with a normal 'add' button which would NOT replace an exiting item.
