The attached patch has been applied to the documentation in package of itclng in Tcl. It allows the documentation to be built as part of Tcl's HTML documentation tree, and to look good once that is done. The only non-editorial part (i.e., that is _not_ formatting) is adding the prefix 'itcl::' to the index line, which makes it much clearer in the generated documentation what is going on by allowing the package names to be included in the contents.
