#130 Adding REVIEW records to a pub is very slow at approval time

Adding REVIEW records to a pub is very slow at approval time. It can cause the submission approval process to itme out when a New/EditPub submission contains a lot of new reviews. We suspect that the process of linking new reviews to reviewed titles automatically is is the culprit. We need to review the SQL logic and possibly enhance it or add a new index.
