#115 Remove references to sff.net and shelfari from the database and the software

Remove references to sff.net and shelfari from the database and the software. The 2 sites are no longer active. Use select count(*) from webpages where url like '%shelfari%' and select count(*) from notes where note_note like '%sff.net%' to check the data.

Also check email addresses. Use select count (*) from emails where email_address like '%sff.net%' to check data. Note: Currently advanced author search does not allow one to search for this (but merging this table/field into webpages using mailto: prefix would fix the search issue).
