#472 Larger "Notes" field and longer data entry fields

In editing a "Publication" notes field is smaller than it needs to be, and makes adding and editing notes more difficult than is necessary. It should be able to be at least as wide as the "Note to Moderator" box, and possibly one line of text taller.

Edit by Ahasuerus: This FR will also be the development vehicle for making data entry fields longer.
