#1703 Errored out submissions should include the approving moderator's user name

Errored out submissions should include the approving moderator's user name. Currently the way the approval process works, it does 4 things in the following order:

  1. Once the "Approve" link is clicked, change the status of the submission to "In Progress"

  2. Process the submission.

  3. Change the status of the submission to "Approved".

  4. Add the user name of the approving moderator and the date/time when the submission was approved.

If the process errors out during step 2, step 4 never happens, so there is no information about the approving moderator or the time when the "Approve" link was clicked.

Ideally, we would want to move step 4 to be before step 2 so that errored out submissions would include the name of the approving moderator. Before we can do that, we need to check if there are any places in the code that assume that the presence of a value in the "reviewing moderator" field or the approval date/time field indicates that the submission has been approved.
