#288 Merging REVIEWs doubles relationship entries

When merging REVIEW records, the entries in title_relationships get doubled. It appears to be harmless since only the first identical entry is used by the application, but we need to clean up the data and the software at some point.

Fixed in the following scripts:

mod/ta_merge.py 1.8
scripts/remove_duplicate_title_relationships.py 1.1 (used instead of     scripts/dedup_title_relationships.sql 1.1)

Installed in r2013-90.
