Reduce number of consecutive errors before killing codec to something more sensible.

Fixed infinite recursion.

Added minimum and maximum target bit rate checks on all audio codecs.

Fixed incorrect bit rate being set on decoder

Added more debugging for tracking down invalid stream errors.

Allow for a number of consecutive errors, e.g. UMC_ERR_INVALID_STREAM, to occur before killing the transcoder.

Added trace log of IPP status code if get encode/decode error.

Merged major update (from 4 years ago) to deal with changing/deprecated API's and then dealing with the total removal of support for the IPP samples.
