#22 Allow ignorables

Many of the weird and wonderful things that people do
with the preprocessor, that cause problems when we try
to parse their headers, could safely be ignored when
building documentation. What we need is a way to tell
the sourcedoc lexer to simply drop specified
indentifiers and not report them to the parser.
