We added FcgidInitialEnv IIIF_VERSION "3" (and did a restart) but the info.json still shows version 2

You need to update your version of iipsrv to the latest Github code (https://github.com/ruven/iipsrv) for this to work. This will all be included in the forthcoming version 1.2 of iipsrv
