Mike,

Most of those you can leave blank. The only things you really need are the name of the web site you wish to monitor (hostname) port (80) and path (/). Actually, the last two should already be set, so you shouldn't need to change the defaults. Method should be already set to HEAD, which is what you want. You could also do "GET" - it shouldn't actually matter.

Once that is configured, go back to your platform monitoring page, and you should see your new HTTP service listed on the left.

And that's it...
