#1823 [HHQ-1833] Create custom notification templates with expande

HQ should allow for the creation of unlimited custom templates that can be selected within the alert definition. This template would define the verbage and structure of the message headers, subject and body, as well as the placement of data tokens wherever deemed necessary.

The token data should be expanded to include things like:


   - sampled data that trigger alert


   - time last / first escalation point for the given alert


   - details of current escalation point, such as other roles/users notified


  - time until next escalation point


   - time remaining for user to acknowledge alert


  - number of previous alerts sent for the current condition


  - time of current condition first entered alert state


  - name of containing resource


  - name of containing platform

There could also be conditional elements, such as special text in the subject like "Alert triggered more than 3 times" if # alerts is > 3, for instance.

Fancy HTML stuff


- Add you own images, like your company logo


- Include a jpeg of a graph of sampled data over some previous interval you define


- Acknowledge / Disable /Escalate Immediately buttons included in email
