#43 hop tweak causes file removal

Have a closed change, and try to tweak in a file that
isn't in that change. Tweak will add the file and
increment the revision, but cvs will not add the change
because the file hasn't been modified.

End up with the hop revision being one higher than the
cvs revision, which causes the file to be removed when
the change is applied.
