Preparing for some possible slow (interpretation) execution of ARM THUMB code.

Adding commented out big-endian hack for barearm.

Hacking in support for ARM THUMB in the unassemble command.

Halt if someone attempts to switch ARM endianness at runtime, since the rest of the emulator is not prepared for that.

Fixing console.cc so that it doesn't complain about buffer overrun when pasting a lot of data.

The left and right triggers are 0 when not pressed, not 128. (According to my real Dreamcast.)

Adding the D button to the Dreamcast controller.

Adding rudimentary culling of clockwise / counter-clockwise polygons in the PVR rendering code.
