#129 Footnote Fixup: Add Reindex From ###

In the Footnote Fixup dialog box, it would be helpful to have the ability to use "Reindex From:" and a box to enter the starting numeric value.

For example, you are processing Vol. II (or higher) of some publication and the Footnotes continue numerically or alphabetically from the previous volume(s).
