#107 Relative track numbering + duplicate filenames

The included patches achieve three effects:

1) They introduces the %T switch, which represent the
*relative* number of the track in the current rip. This
allows, for instance, to rip a concert going from track
4 to track 6 numbering the various pieces 01, 02, 03.

2) They introduces a switch "Allow duplicate names"
that inhibits track name disambiguation. This is
fundamental when ripping classical music (Vivaldi can
put three "Allegro" tempo indication in the same
concert 8^).

3) They eliminate the debug print that outputs the CD
title every time you modify a character.

Just one point: here and there, FillInTrackInfo() is
called with a constant track number argument 0. In
those cases, I used 0 also for the relative track
number. I guess it is right, and it seems to work, but
a check would be a good idea.

    Can you at least comment as to why you did not consider my
    patch for inclusion? I was hoping to find it in new versions
    of grip...
