A fused card is ready for normal javacard/Global platform operations. With an unfused card, if you have the card keys, you can change the underlying OS, native libraries etc. You can also do bulk personalization (e.g. write a card image rather than single applets). To do that you probably need the development kit from NXP. Its got all the fun stuff in it. I don't know what their requirements are to get such a kit. One last warning... a card in the unfused state might have different export (country to country) restrictions than an unfused card due to unfettered access to the core of the libraries.
