OK, here's the current state of the patch. I got rid of get_papersize_by_index() altogether. It was only used internally, and it's inefficient (quadratic to go through the whole list). Going through the linked list works just as well. Thus far, I've regression tested genppd and am testing rastertogutenprint. Still need to spot check the GIMP plugin (in particular, custom sizes).
