For generation of online documents, where you don't
know where the reader is, the so-called PA4 paper size
is rather handy. It is defined as having the width of
A4 paper and the height of U.S. "letter" paper, so that
it can be printed on either without any funnies.
I think adding this paper size to the relevant places
in GhostScript would be a good idea.
The name PA4 comes from an ISO draft, which was
(unfortunately?) later withdrawn. It's the closest
thing to a standard name for this "virtual" paper size.
