[generguide-devel] Uncommented ENTITIES in guide docs

I've changed all <!--&entity;--> to &entity; in the generguide docs.
I see this as an interum measure until we work out how we are going to handle 
variables.

Of note, I think this now means that we don't have a free WYSISYM editor which 
will process generguide docs.  This means we are failing one of the primary 
use cases for the generguide project.  (I have to confess that I have not 
done much research trying out the other free docbook editors).
