Oops,
I sent this email to the wrong list.
No wonder Artur has not responded.  :)

Anyway, I've gone ahead and updated the bin/build.sh script to convert 
commented entities to uncommented entities.

I've created a new file: guide/resources/entities.ent for storing generguide's 
entities.

I've also got a response from the XXE team saying that entities and 
<xi:include> functionality should be supported in next release (before 16 
August).  I'm not sure if the functionality will be in the free version.

Next, I plan to go and remove geotools specific information from the guide.
