[Algorithms] lens blur (or Bokeh filter)

Hi,
I'm trying to find out how lens blur algorithm is working. For example
it is implemented in Photoshop:

it's also known as Bokeh effect:
http://en.wikipedia.org/wiki/Bokeh

Does anyone know how can I implement this algorithm? Any links will be
highly appreciated. Failed to find anything useful in google.
