One of the things I've found that is important for the quality process, is that bugs should be closed, if possible, by the person who opened them, or - if that person isn't available - someone who will take responsibility for testing the bug fix to make sure the bug can be closed. This makes it a lot less likely that the same bug will keep coming back over and over again.
