I would really appreciate if you had also kept the copyright statement and my name...

Hi,
I'm sorry but this file was probably produced before 1999 and I have no way to link it to any Open Source project at that date.
Given the simplicity of the code, I do not see why the original author would have needed an existing code base to code this very simple plugin.
