#1 Fixing some Undefined Property and Variable issues

I am attaching a patched version of
Formitable.class.php where I added a few lines to
correct errors I was receiving in my logs due to
improperly registered globals. All changes are
preceeded by comments that begin with "FF:" for
searching purposes. Lines affected are (in my
attatchment) 19-20 and 56-58.

Simple stuff but I prefer the simple stuff not be
overlooked. Turn off register-globals in your testing.
