The `fmtt` formatter SVN

- FIX silly omission in `lkindname()`.

- FIX stack overflow with fixed lines.
- FIX much better space-out feature.
- NEW substitute `\U+0F0F` unicode point escapes.
- NEW when `-n` option, rewrite gap after section number to SP , NBSP (and recognize these again, of course).
- FIX ridiculous error in `sreplace()`.
- NEW when `-m` option, pass _Pandoc_-style header through as fixed.

Add keywords to `src/` dir; add ignores to `VS2003`.

Update the VS 2003 project files to include the proprocessing tool `ctxgen`.

Update VS 2008 project files to include the preprocessing tool `ctxgen`.

Place digraph pattern preprocessing/sorting into external program. This simplifies the context matching stuff at runtime a lot. Context patterns are now defined in a text file, processed into C array definitions and then included into fmtt at compile-time. Is kind of working, but there is still some work to do.

... yet another silly error fixed.
