The `fmtt` formatter SVN

Extend patterns for `-D`.

Define RFC 1345 digraphs for (most of) ISO 8891-1.

Working on '-D', 'd [char]' Options.

Update `version.h`.

Add project files for VS 2008 (MSVC 9).

Add options for usage and version/license information.

Remove VS filter group "Header Files": There are no header files in `fmtt`.

New option `-g` to respect Markdown conventions:
1. '##' style headers (new),
2. definition lists (as before),
3. fenced code blocks (as before).
NOTE that 2. and 3. are now *require* the `-g` option set, but were formerly by default.

Change `-r`<num> option to mean "<num> characters or more" (instead of "more than <num> characters").

Add option `-Z` to protect Z Notation (e-mail mark-up) from line breaking.
