Update the testlog after integrating the unicode branch.
This includes many new tests and some new failures.

Ingrate the "unicode" feature branch into trunk.

Fix the file test to run on Windows.
On Windows, perl's chmod function cannot remove read access, so we use cygwin's chmod instead.

Fix a bug where if OPENAPPEND couldn't read the file, it threw an error but still opened the file with an indeterminate encoding.
Now it throws an error and doesn't open the file.

Merge in changes from trunk to get GNU/Linux implementation of SHELL
