Fix a bug in the translation table generation script that treated a
COPYDEF "FORWARD "FD within the Italian SLOWDRAW implementation in the
startup-1040.logoscript as an *Italian* translation of FD.

Add navigation links to the appendix when it includes translations for color names.
This was done at the request of Francesco Simone Carta.

Use native line endings for fop.xsl, since it's a text file.

Update the path to the docbook.xml in fop.xml to match its location in a recent version of cygwin.

The new path was in the file but commented out, so it might have been the path used on Fedora Core and now cygwin has finally caught up.

Fix the nomenclature in the translation tables to refer to commands as "procedures",
since they translated both commands and operations. The distinction is enforced
everywhere else in the manual, so it's good to eliminate the slopiness in terminology
from the non-English versions.

This revision also changes the index term for the procedure name translation tables to
follow the pattern of the color names. This makes the different pages self-apparent
in the index.

Fix the order ("operation" was not in the correct place alphabetically)

Add definitions for Form and Special Form.
Also change "chunk of Logo code" to "series of tokens" to sound smarter.

Fix a non-impactful bug in the regular expression which extracts translations from COPYDEF instructions.
The regular expression incorrectly counted a ] that ended a CATCH block as part of the translated procedure name.

This was non-impactful because only the German startup.logoscript used such a construct and the German FMSLogo doesn't use the generated translation tables.

Switch from blocking undocumented procedures to only allowing documented procedures in the translation tables.

While this makes the code cleaner and more correct, it was primarily motivated by the fix for Bug #493, which put new COPYDEF calls into startup-1040.logoscript that were part of the implementation of SLOWDRAW, and not for translation purposes.

This also fixed a few mistakes in the Portugese and German translation tables, but since these use a custom manual, the translation tables were ignored.

Further refinement on fixing Bug #493; the body of SLOWDRAW is now copied into startup-1040.logoscript and then COPYDEF'd to RALLENTA. This addresses an issue where mixing RALLENTA and SLOWDRAW calls resulted in an error when the AVANTI was later run.
