[Firebird-docs] Issue tracker moved

The issue tracker for the Firebird documentation has been moved to 
https://github.com/FirebirdSQL/firebird-documentation/issues

I have also gone over all the existing open issues and closed those that 
no longer seemed relevant (or were already done).
