As for question 2:
Everything goes into the trunk, or whatever it's called in Git. Publishable documents go into B_Release as well. This branch was already present in CVS.

Your first and last questions should be answered by Mark. Or I could give it a shot and then if you do the exact opposite of what I tell you, it might just be right ;-)
