If the USING keyword is used, it looks that:

  a) I can use my own (already exisiting) index for a constraint
  b) If I omit the subclause a constraint will be created without an index

Now both above statements are wrong. I understand that these features may be
useful and have already been requested, but I'm not going to implement they
for v1.5.
