This has been a longterm issue. I just updated to the latest dev. build and it continues.
This is the Windows 7 Ultimate x64 Explorer message on crash (Intel CPU, AMD GPU):

It references the dev build version I installed, so I assume that the issue still exists.
It happens only if I have an explorer view setting that shows previews, and not until I've scrolled down a ways (presumably to a point where a problematic preview is attempted.)
Thank you, wonderful developers :)
Add explorer.exe to the blacklist (Options > DirectShow control).
This shell extension is much better than the standard one from Windows:

My world is now perfect... better, even. :)
However, I'm still guessing that ffdshow.ax terminated improperly or something, and the issue probably effects lots of Windows users.
Now, why did I wait YEARS to ask this question?! :) have a great day. :)
