I schedule fetchmail from cron and run it every 5 minutes during the day
and every 20 minutes overnight. On average, I have to wait 2.5 minutes
for an email to arrive. That's plenty fast enough. If I'm on the
phone with somebody and know they've sent the email and I need to look
at it during the conversation, I just kick off a manual fetchmail run.

If you need real-time delivery, use a telephone. I my brother emails me
a picture of his flower garden, do I really, really care if it arrives
in 30 seconds or in 5 minutes?
