I had the same problem at an early revision of Caldera Open Linux 
(2.2 I think). Though in my case, I had the problem with expect/tcl, not
expect.pm.

The OS was not configured to grant additional pty's (psuedo terminals) to 
non-root users.

I suspect this may be a kernel configuration issue for Irix, but I have no
experience with that Unix. I believe that this was the solution to the
problem on Caldera Linux, but I didn't have to go down that path as I
upgraded the OS, and the problem went away.

You may be able to find a quick answer by searching for the keywords
"Irix" and "pty" on the net.
