Hi Gary, I think you should take a look at Joe Wicentowski's materials for the Digital.Humanities@Oxford Summer School 2011, at <http://digital.humanities.ox.ac.uk/dhoxss/2011/sessions.html>, down towards the bottom of the page. The format of the Punch app is not that used in eXist-db now, but Joe's setup will work fine, and it shows how to make TOCs.
