The driver setup is almost exactly from the current intel driver.
If you could do a tcpdump, or use a similar network sniffer and confirm
the dhcp packet is not actually sent that would say something.
My best hunch is that you are not specifying a filename in the dhcp response.
Until I have evidence to the contrary I believe you have not properly configured your
server.
