Hi,

here's a patch (against etherboot 5.0.5) of mkQNXnbi, which I
contributed a couple of years ago.
In the meantime, I've installed it at two sites for a total of appr. 30
diskless QNX4 workstations.
I've also had to answer questions from a surprisingly high number of
other Etherboot users and thereby learned that my documentation was
slightly flawed.

Here's my attempt to fix the docs and examples.
