#12 Links outside the docs should open in the top frame

Links to URLs that don't point to other locations in
the documentation should open in the top frame, not the
current frame.

When a reStructuredText link like this:

----------------------------------------------
Some text that refers to `something else`_.

.. _something else: http://www.example.org/
----------------------------------------------

the anchor that that gets generated should include a
target="_top" in the moduleListFrame and moduleFrame,
and target="_parent" in the mainFrame. (Using
target="_parent" in the mainFrame should always produce
the right result, while the other two frames are a
compromise since what's really desired is the grandparent.)
