#137 Example and documentation of chained ODD

Following on from discussion of ODD chaining in [feature-requests:#119] I would like to create an example customization of EpiDoc, along with some basic documentation, to show people how to add elements, attribute values etc. that have not been approved by the EpiDoc community to their local version of the schema, without losing the richness of and later additions to the EpiDoc schema.

For example, this was discussed under [bugs:#174]. I would like to get this working as soon as possible after release 9. next week.

    I wasn't able to get this working locally, when I tried it last year (possibly due to refusal to RTFM), and @hcayless and I never found time to sit down together and look at it. Would someone else like to take over this ticket? Otherwise it won't get done by milestone:9.2.

    @filosam and @alessio will give @gethia an example of two of ways that SigiDoc customised the EpiDoc schema, and she will produce a working and documented chained ODD that implements that. She will then pass the ticket on to @alessio, who will produce a complete SigiDoc chained ODD.
